The competent person shail have the following certification: iii COSH Training Certificate from DOLE or its accredited safety training organizations; iii Must be a holder of TESDA prescribed Scaffold Erection Certificate; and ii.ji At least 2 years’ experience in scaffold erection. b. Scaffold Erector i. All scaffolds erectors must undergo the standard scaffold training and assessment prescribed by DOLE and TESDA; and ii. Scaffold erectors shal! nave the following certifications: iii One Day Workers Safety Organization from DOLE or its accredited safety training organizations; and ili. Must be a holder of TESDA prescribed Scaffold Erection National Certificate. Y. Machine Shop Safety 1. 2. Allow only Authorized Personnel are allowed to use the machine shop; Use required and appropriate PPE while working in the shop in accordance with Section IILA.2 hereof, . Observe the “buddy system” while working in the shop; . No eating, drinking and smoking inside the machine shop area; . Keep the work area clean and always keep the floor free of grease, oil or any other liquids; . Do not allow horseplay and/or running tn the machine shop; . Do not wear loose jewelry and clothing including long sleeves and ties in the shop; Al === dc2021-06-0016-pages-42.ppm === 8. Tie back long hair while working in the shop; 9. Seek for your co-worker's assistance in handling large, long, or heavy pieces of material or machine attachments; 10.Report any unsafe acts and conditions to the shop supervisor and/or safety officer; 11.Always make eyewash and first aid kit station available in a designated place; and 12.Post the machines users guide and/or safety manuals in a strategic location for easy reference. . Tools, Equipment and Machinery Operations 1. Workers must only work with tools, equipment and machines that they have been authorized to use; Appropriate tools, equipment and machines shall be used on a specific job; Each tools, equipment and machines must be examined before use. Damaged toois, equipment and machines should be reported to proper authorities; . Damaged tools, equipment and machines must be repaired before using it; All tools, equipment and machines necessary to complete the task must be kept in good condition with regular maintenance: . Tools, equipment and machines should be operated according to the manufacturers’ instructions; . All tools and equipment should be kept cleaned and stored appropriately when not in use; All portable electrically-driven tools, equipment and machines shail be properly grounded before use; . tron or steel hand toois may produce sparks that can be an ignition source around flammable substances. Where this hazard exists, spark- resistant tools made of non-ferrous materials should be used where flammable gases, highly volatile liquids, and other explosive substances are stored or used; 10.Air supply shall be shut off when pneumatic tools, equipment and machines are notin use; === dc2021-06-0016-pages-43.ppm === ‘Ne Ne 11. Pointed or sharp tools, equipment and machines shali be provided with cover; 12. Talking to others while they are operating a machine is prohibited; 13. Tools should not be left unattended. They should be placed in the designated toolboxes or cabinets after using them; 14. A machine should not be left unattended while it is running; 15. Only one person may work on a machine at a time; 16.When working with another person, only one should operate the machine or switches; 17.A cutter should not be used by pushing it towards your body parts; 18. Work piece must always be secured with a clamp or vise; 19. Compressed air should not be used without a safety nozzle to clean the machines. Compressed air should never be used to clean clothing; 20.Metal chips, turnings or shavings shouid not be removed with your hands; 21. Tools should be disconnected when not using them, before servicing and cleaning them, and when changing accessories such as blades, bits, and cutters: 22.Proper apparel for the task should be worn. Loose clothing, ties, necklaces, or jewelry can become caught in moving parts; 23. Electrica! hand tools shai! not be used in a combustible environment until the Supervisor certifies that conditions are safe and a Hot Work Permit has been issued; and 24. Tools, equipment and machines user's guide and/or safety manuals should always be followed. AA. Machine Guarding All moving parts of prime movers, transmission equipment and all dangerous parts of driven machinery shall be effectively guarded, unless so constructed or located to prevent any person or object from coming or brought into contact with them: 1. No person shall remove or make ineffective any safeguard, safety appliance, or safety device guarding a dangerous machine or machine part unless such is authorized and the machine is stopped for the 43 === dc2021-06-0016-pages-44.ppm === BB. purpose of immediately repairing and adjusting such machinery, guard, appliance or device; 2. Warning signs with standard color shall be installed near the machine being repaired or its guards removed; 3. Upon completion of the repairs or adjustment, such guards, appliances or devices shail immediately be reinstalled before the machine is used; 4. Guards shall be designed, constructed and used that they will: a. Provide positive protection; b. Prevent all access to the danger zone during operations; c. Cause no unnecessarily or inconvenience operation or production; d. Operates automatically or with minimum effort; e. Be suitable for the job and the machine; f. Cause no obstruction or interfere with machine oiling, inspection, adjustment and repair; Withstand long use with minimum maintenance; Resist normal wear and shock; Be durable, fire and erosion resistant; Not constitute a hazard by themselves; and Give protection against operational contingencies and not merely pratect normally expected hazard: xo >a 5. Standard guards or enclosures shail be made of materials suitable for the purpose for which they are designed and constructed; and 6. All machinery guards shall be securely fastened to the machine or to the floor, wall or ceiling and shall be kept in place whenever the machine is in operation. Biological Safety 1. General Requirements a. All field activities/works shall be covered with appropriate work permit system; b. Required PPE shal! be used while doing field activities such as long pants, long-sleeves shirts, shin guards, heavy high top shoes or hiking boots to cover exposed body parts and provide some protection and use walking sticks; and c. Workers training should always include information on safe methods and prevention for highly hazardous procedures that are commonly encountered during outdoor field work. 2. Workplace Inherent Biological Hazards 44 === dc2021-06-0016-pages-45.ppm === a. TO avoid bees and wasp stings: tii. iv. Vv. Vi. Avoid known areas of concentration such as hives and nests; If flying insects are around, leave the area and refrain from swatting at them; Avaid sugary foods, drinks, and strong fragrances or perfumes because some insects may be attracted to them; Wear long pants, and long-sleeves shirts; Wear heavy high top shoes or boots, protective gloves, and use walking sticks; and lf stung by bees or wasp, seek immediate medical attention. b. To avoid snake bite: i. Vi. Vil. Vill. Always ask about snakes from local residents and employ local resident as a guide; When walking, keep your eyes on your path and avoid stepping into clumps of vegetation: Do not step over logs or large rocks if you cannot see over them; Wear long pants and long-sieeves shirts; Wear heavy high top shoes or boots, protective gloves, and use walking sticks; Do not kill non-poisonous snakes, they keep the food supply low and keep the population of poisonous snakes down; Never pick up an apparently dead snake with your bare hands. Even severed snake head can inflict a deadly bite for 15 - 30 minutes after separation from tne body. Shouid it be necessary to move the dead snake, use a stick; and if bitten, seek medical assistance at the nearest clinic or hospital, vials or antivenin shall be approved and supplied by the Research Institute for Tropical Medicine (RITM). c. To avoid feeches: it. Wear long pants and long-sleeves shirts: . Wear heavy high top shoes or boots, protective gloves, and use walking sticks; and During rest, find a spot with direct sunlight, leeches do not exist in dry and hot places. CC, Geohazards Requirements 1. The Employer shall establish a geohazard assessment tearn and mandate: 2. The team shail identify the presence of geohazards within the contract area and/or nearby vicinities, thru the inventory of landslides, flooding, and rock fall prone areas; 45 === dc2021-06-0016-pages-46.ppm === 3. The Employer shall initiate mitigating engineering and administrative measures and formulate emergency/contingency plans; 4. The team shall meet annually or as the need arises; 5. All Workers and the communities shall be properly informed of the major geohazards, thru posting of illustrations in the form of schematics (i.¢., maps, posters, signages and drawings) in public places; and 6. In the occurrence of geohazard events such as landslides, earthquakes and floodings, the Disaster Emergency Preparedness/Contingency Plan and Response Team shail be deployed to the concerned area. Section IV. Power Plant Facilities A. Power Plant Safety 1, oO All Workers shall be qualified and trained in their respective job assignments; All Workers shall undergo occupational safety and health orientation and training; All Workers shall immediately report any observed unsafe condition, incident and accident in accordance with the established company safety reporting procedures and quidelines; All Workers shall be required to wear appropriate and prescribed PPE in accordance with Section HLA.2 nereof, Only Authorized Personnel shall conduct repairs and calibration to any plant equipment and instruments; Ali power plants shall establish work procedures for: i. Start-up and Shut-down; ii, Emergency; ii. Normal and Abnormal Operation; and iv. Operation and Maintenance Work for the following: Pressure Vessel; Pumps; Condensers; Cooling Towers; Turbines and Generators: Electrical and Instrumentation; and o Fans and blowers. oO O Aa 4 Such procedures shall include hazard identification, analysis, mitigation and emergency response; and 46 v === dc2021-06-0016-pages-47.ppm === 7. All works shail be covered with appropriate work permit system. B. Control Room Safety 1. Only Authorized Personnel are allowed to manipulate control panel knobs, buttons, and switches. Bystanders are not allowed inside the control room unless authorized; Air conditioning unit should be kept running twenty-four (24) hours a day to maintain the control room temperature and to keep the air clean and filtered; . The door of contro! rooms should always be closed to reduce outside air infiltration/cooling loads and H2S contamination; Control rooms are “NO SMOKING’ areas and such policy shall be observed ait all times; . LOTOTO procedure and proper clearance from the authority shall be strictly complied with; Automatic/emergency fighting system shall be provided at all access and means of egress; The door of circuit breaker/panel should always be kept closed; Coniroi and power meter panel shali have safety floor markings; and Emergency button of the control panel shall have interlock and cover to prevent accidental operation and would cause force shutdown of the power plant. C. UPS and Battery Bank Room Requirements 1. 2. Only Authorized Personnel are allowed to enter the room; Air conditioning unit must be operated twenty-four (24) hours a day at temperature not greater than 15°C for electronic equipment cooling; . The door of UPS and battery bank room should always be closed to reduce outside air infiltration/cooling loads and H2S contamination; Battery bank should be kept free from leaking solutions; . Battery room should not be used as dressing room; . The exhaust ventilation must be placed in accordance to PSME Code. A spare fan should be readily available; 47 === dc2021-06-0016-pages-48.ppm === The direct current (DC) system shall be readily available whenever the DC power is interrupted to the system; Eyewash station must be provided near the battery room; . Check-up of battery electrolyte teak and specific gravity should be done regularly; and 10. Proper handling, storage, and disposal of unserviceable batieries should be observed in compliance with regulatory requirements. D. Equipment Repair and Troubleshooting in Electrica! Power Facilities 4. o sl Never attempt to execute troubleshooting and repair of any line conductors and electrical equipment and circuitry at control panel board without permission and proper coordination with area supervisor: . Always wear electrically rated safety shoes when performing troubleshooting and repair of any faulty line conductors encased in metallic cable tray: Place warning tags (indicating reasons for isolation) on any circuit cut-out/breakers with the requested “DE-ENERGIZE’ line associated; Insulate all dangling and bare terminals of power conductors right after disconnection; De-energize all power line conductors and use voltage detector to ensure the line is safe, when troubleshooting and repairing alll electrical works; Ensure all electrical tools/equipment used are properly insulated and must be capable to handle the voltage of the work piece; Conduct insulation resistance testing on any line conductors and equipment being repaired and/or replaced prior to re-energization to determine the di-electric strength of the insulation; and Advise superior immediately any faults, tripping or hazards observed on any electrical equipment or installation. Section V. Geothermal Energy Operations A. Geothermal Energy Operation Requirements All geothermal energy operations shall comply with the following minimum requirements: 48 === dc2021-06-0016-pages-49.ppm === > a>oaoo® Standard Operating Procedure; Orientation/Training; Work Permit System; Close Supervision; Monitoring Tools/Instruments; Communication Tools/Equipment; Emergency Preparedness and Disaster Management Plan, if necessary; and Personal Protective Equipment. B. Pre-development/Exploration Stage 1. General Requirements a. Make sure only Authorized Personne! shall be allowed to conduci geochemical sampling and/or geophysical survey; Designate an Authorized Personnel, with minimum Basic Occupational Safety and Health (BOSH) training with DOLE- Accredited STO, as Safety officer during the entire geological, geochemical sampling and/or geophysical survey, duly registered with the DOE: Conduct geohazards assessment prior to pre- development/exploration activities; Conduct daily safety meetings to review the project, personne! deployment locations, equipment layout and the Worker's condition; Check the condition of the service venicile, communication tools and PPE; Coordinate with local authorities for security purpose before starting pre-development/exploration activities. 2. Composition of a Geothermal! Exploration Team a. One or more geologists, geochemists, geophysicists and field technicians, ensuring the use of a buddy system: One lead guide, knowiedgeable with various terrains and bodies of water, who will ensure that the trail is safe before proceeding; One team member who shail function as sweeper; Helpers, for rolling camp set-up; and An advance team shall be formed to scout and assess the exploration area for accessibility prior the entry of the main exploration team. 49 === dc2021-06-0016-pages-50.ppm === 3. Dealing with Various Kinds of Terrain a. Under the responsibilities of the lead guide, confirm the accessibility of the area; b. For passable terrains, check for falling debris, sharp objects and maintain a distance of two (2) meters in between individuals; c. For passable terrains with the aid of rope, setup the rope on sturdy trunk and pass one at a time with caution; d. For impassable, such as loose ground and/or nothing to hold to, look for alternative route; and e. The team shall avoid location of traps. If quick sands and aggressive animals are observed, the team must plan a reroute. 4. Dealing with Bodies of Water a. The team shall consider the various types of bodies of water when traversing or crossing through it: iti. For caim and mid to shallow bodies of water, the iead guide shail assess the slippery areas and sudden changes in water depth, aided by a stick. Then, the team shail thereafter pass one or two at a time aided by the guide; For raging mid to shallow bodies of water, the lead guide shall check for rock boulders as path. If available, the lead guide shall check for slippery areas and proceed with caution. If none, the lead guide shall tie and secure the rope, in a position oblique to the flow. The team shall pass one at a time aided by the guide, holding tightly on the rope; and For deep but calm and/or raging bodies of water, the team shall re-strategize to a safer method of crossing using various means of water transport (i.e., banca, canoe, balsa, etc.) or re-route to a safer path. 5. Camp Safety a. In selecting camp location, the following shail be considered: ii. iil. Camp location shail be at least 10 meters away from bodies of water; The camp area shall be clear from sharp rocks, glass shards and poisonous pianis; The camp area shail be slightly sloping to ensure water drainage; The camp area shall have a protection from strong winds; === dc2021-06-0016-pages-51.ppm === Vv. Vi. vil. The camp shal! not be set up too close to the water source to be considered safe from wild animals; Cooking shail be done far from the tents (~10m away) to avoid inviting aroma to wild animals; and Food stuffs and other aromatic materials shall be stored outside the tents. If possible, hang the items at a height not reachable by animais. b. Other safety protocol: i. ii. iti. Vi. Insect repellent should be applied as deemed necessary; The buddy system shall be implemented continuously, whereby measures ensuring that the location of one buddy is always be known by the other are in place; Camp fires are discouraged. However, if such is absolutely needed, the team must ensure that camp fires are properly put out before the campers go to sleep; During night time, at least two (2) persons shall be assigned as look out on a routine basis to oversee the safety and security of the camp site; Alcoholic beverages are prohibited during camping; and if possible, consuming and/or damaging local flora and fauna should be avoided. . Outcrop Observation a. For data collection: il. In observing the outcrop from afar, one shall position himself on stable ground, visible to the team members; and In measuring structural orientations, the following must be done: o The team should be aware of falling debris and unstable areas prior to getting near the outcrop; and o The lead guide shail clear the outcrop from plant hazards and unconsolidated debris. b. For rock sampling: i. ii. IV. Vi. The team shall wear proper PPE prior to hammering; Other team members should also wear goggles. Otherwise, they should position far from the point to be sampled; Prior to hammering, the geologist or sampler shall ensure that no limb is situated immediately below the rock hang; For large rock fragments intended to be broken into pieces, the sampler must practice hammering with rock stabilized only by foot (standard geo practice); The edges of the rock sample should be smoothened prior to placing them on the sample bags; and Filled sample bags shail be placed in a heavy duty sack or bag. 5] av === dc2021-06-0016-pages-52.ppm === 7. Thermal Sampling a. Only Authorized Personnel are allowed to approach the thermal areas; The sampler must ensure that his/her position is on stable ground before commencing the sampling. He/she must ensure that the area is cleared of other hazards such as falling debris; and Conduct multigas reading. At H2S levels >10 ppm and CO levels >35ppm the sampler is required to wear a SCBA unit. C. Development Stage 1. a. b. C. d. e. 2. a. b. Fluid Collection and Disposal System (FCRS) Maintenance Only Authorized Personnel shall operate geothermal wells and FCRS; Appropriate PPE should be used in accordance with Section IILA.2 hereof; Opening and Closing of wells to the FCRS and Re-injection (RI) System shall be done in accordance with LOTOTO procedures as described in Section HI.G hereof; Valves in FCRS shall be assured through a Preventive Maintenance Program (PMP) which include regular opening and closing where appropriate or stroking as described in Section V.C.5 hereof: and Workers entering an area around discharge facilities (e.g. rock muffler, silencer, diffuser, rupture disc, etc.) shall be properly informed of the hazard (e.g. splashing of hot fluids, noise, H2S and other hazard that maybe present. Abrasive Blasting Guidelines Blasting equipment shall follow Operation and Maintenance guide: Appropriate PPE should be used in accordance with Section Ill.A.2 hereof; Blasting agent must be dry and free from unwanted items; Blaster shail be provided with fresh air supply for breathing and ventilation; Blasting operation shall be done in such a way as to prevent exposure to other personnel not involved in the operation; 52 === dc2021-06-0016-pages-53.ppm === f. g. The compressor operator must be in close coordination with the blasting operator; and The relief valve or plug of abrasive blasting equipment must conform to the standards. FCRS Valve Repair/Replacement a. h. All valve replacement shall be done in conformity with the manufacturers and company standards: Appropriate PPE should be used in accordance with Section NILA.2 hereof, Isolation of valves for repair or replacement shall follow the LOTOTO procedures as described in Section III._G hereof, A self-contained breathing apparatus shall be stationed in the area for rescue purposes; If a significant level of poisonous gas is present and persistent, industrial blowers shail be used to lessen and prevent gas accumulation within the area. In case H2S level is above 10 ppm, the work shall be suspended or deferred for remedial action; Replacement of the wellhead master and wing valves shall be done in accordance with company standards to prevent blowouts: Proper coordination between workers shall be observed to prevent injuries and untoward incidents: and Appropriate equipment shall be used for lifting purposes. Valve Packing/Injection Requirements a. D. Ensure that trapped body pressure is properly relieved to zero; and Do not exceed packing injection pressure the rated pressure requirement provided in the manufacturer's specifications. Wellhead Valve Servicing/Stroking a. Prior to wellnead valve servicing, all personnel shail follow requirements in Section V.A hereof, Safety remarks on work permit shall be carefully observed whether the valve can be stroke or not; LA Go === dc2021-06-0016-pages-54.ppm === C. There should be stroke or exercise wellhead vaives based on scheduled PMP. Stroking or exercising of wellhead shall be done in close coordination with the authorized operations personnel: Defective master valve shall not be stroked/exercised and must be replaced; and Stroking activities shail not be done during bad weather or when gas is beyond exposure limit. Drain Valve De-Cilogging a. De-clogging equipment/devices must be inspected and checked prior to use; and De-clogging activities shall conform to company’s standard operating procedures. Entry to Vessel/Header/Silencer Requirements a. i. Hazard assessment shall be conducted prior to entry to any vessel/header or silencer: . Entry to vessel/neader/or silencer must conform with the established company Confined Space Entry Procedure (CSEP): isolation tags, chains and padiocks shall be instalied on the respective servicing valves to notify and prevent accidental opening: . The inside of the vessel/header or silencer must be free of leaks or gas accumulation before a person is allowed entry: . Before opening any enclosure, there should be a confirmation that the pressure is zero; No entry shall be permitted in case only one manhole is opened or when there is only one opening; otherwise; the use of industrial blower is needed; Personnel shall be equipped with life or tagging lines prior to entry; Personnel working at height shall securely fasten or tie the tools to a rope string to prevent injuries or property damage when they fall or drop; A confined space entry watch shall be stationed at the access whenever personnel are inside the vessel, header or silencer; Ensure that vessel is leakage free by installing a skillet blind flange and must be well ventilated; 54 === dc2021-06-0016-pages-55.ppm === 8. k. Rotation of personnel working inside vessel shall be observed; |. No compressed gas cylinder shall be allowed inside the vessel: m. Only explosion proof tools and lighting shall be used when working inside the vessel that previously held flammable liquid or gas; and n. Supervisor shall accurately account personnel and tools that maybe left inside the vessel before resealing. Pressure Relief Valve (PRV) and Rupture Disc Operation Requirements 10. a. Appropriate safety sign shall be installed at the PRV and rupture disc; b. Unauthorized personne! shall not be allowed to enter within the vicinity of the relief valves and rupture disc; c. Appropriate protection shall be installed at the tip of the exhaust stack of the PRV and bursting disc. These shall be covered with light materials to prevent rain or foreign objects from going in; d. Rupture disc and PRV design must be according to the design specification. Rupture disc and PRV design must be according to the design specification; and e. To prevent accidental isolation, appropriate locking device must be provided. Motorized/Pneumatic Vaive Calibration Only Authorized Personnel shall be allowed to operate, stroke or calibrate motorized and/or pneumatic valves. PRV Testing a. PRV shail be properly calibrated and certified by the manufacturer before installation; b. PRV shall conform to company’s Preventive Maintenance Program; c. PRV lift and reset pressures shall not be altered or changed without approval from Authorized Personnel; and d. Alteration or changing of the lift and reset pressure of PRV shall have prior approval of the authorized approving officer from the FCRS operations. Maximum settings shall not exceed the manufacturer's recommended set limits. === dc2021-06-0016-pages-56.ppm === 12. Weil Start-up Requirements a. Notification of personnel and community shall be conducted prior to Start-up of wells b. Only proper tools and equipment shall be used; c. Work site evaiuation shall be conducted and necessary corrective measures shail be implemented prior to start-up of wells; d. Appropriate pressure relief system shall be installed; and e. Release of trapped gases shail be properly managed. Gauges Maintenance a. Gauges and accessories to be used shal! conform with the required specifications; b. Installed gauges shall be properly calibrated; and c. Proper tools and equipment shall be used when replacing gauges. Section VI. Geotherma! Wel! Maintenance Requirements The following well maintenance activities shall comply with the requirements stated in Section V.A. Master Valve Replacement: Downhole Measurement; Well Discharge / Testing; Well Blowout Prevention; Wellhead Valve Servicing/Stroking; Geothermal wellhead equipment replacement tool; and g. Non-rig activities (e.g. broaching, acidizing etc.) “om aoc Section VII. Rig Safety A. Slips and Elevator 1. 2. Slips and elevator shall not be modified: Keys, pins, dies, handles and bodies on all pipe and collar slips shall be checked frequently for wear; Rig personnel shail keep their hands and feet, as well as chains, ropes, etc., away from the rotary table when it is in motion; Slips shali be properly positioned when not in use; === dc2021-06-0016-pages-57.ppm === 5. Slip handles shail be used to manually raise and lower slips. Handles shall be grasped with palm up; 6. Atleast three workers shall pull slips using proper techniques; 7. Latches, latch springs, hinge pins and elevator shoulders shall be inspected periodically and maintained as necessary; 8. Riding the elevator as a means of personnel transport is prohibited; 9. The elevator ear locks shall be fitted with steel bolts of proper size; and 10. The elevator or bails shall not be grasped in the area of the link eyes. B. Catheads 1. When in operation, the driller or assistant driller shall be at the driller's console to disengage and stop the cathead; 2. Worn, damaged or grooved catheads and worn dividers should be repaired or replaced; 3. Catheads shall be equipped with anti-fouling devices (Rope Divider) at all times. The recommended clearance between the cathead friction surface and the Rope Divider edge is 6.5mm (1/4"): 4. A swivel and safety hook attached to a lifting cap shail be used when catlines are used to pull pipe through the V-door; 5. Only the amount of wraps necessary to raise the load should be used; 6. Objects shall be raised and lowered at a safe pace and the wraps removed after the load is landed. The locking device shall not be used to suspend a load: 7. The rope or fine shall not be left wrapped on or in contact with the cathead while itis notin use. The rope shall be safely stored when not in use; 8. Rig personnel shall not be lifted into the derrick by use of the catline: 9. The rope shall not be wrapped around the operator's hand when in use. Do not stand inside rope coiled on floor; 10. Except for specially spliced ropes that are used for spinning casing, a rope spliced or a frayed part of a rope or line shall not be used on a cathead; and 11. A flagman or two-way communication device should be used when the cathead operator cannot see the object being raised or lowered. 57 === dc2021-06-0016-pages-58.ppm === C. Air Hoists —" ad All air hoists shall be equipped with a drum guard and line guide; All hoisting lines shall be inspected periodically; The hoisting line shall not be in contact with any derrick member; The recommended load capacity of the air hoist and wire rope shall not exceed the manufacturer's recommendation; Rig personnel shall stand clear of a suspended ioad; The air hoist operator shall set the drum brake anytime a load is in suspension. The operator shall not leave the hoist unattended; and Use the air hoist for transport of personnel. Provided that this will require appropriate authorization from Supervisor or any managing personnel. D. Pipe Tongs and Lines 1. Make up and break out shal! be provided with lines of sufficient size and length; Tong counterbalance and parts thereof shail be restrained, guarded, or located as to prevent them from falling on or striking crewmembers: Makeshift weights such as tools, pipe. protectors, bits, etc., snall not be added to the counterbalance weights; Tong die siots shall be properly maintained; Tongs shail be provided with sharp dies at ail times and properly pinned in die slots; Die drivers shall be used to remove tong dies: A hard hat, full-face shield, safety glasses and protective gloves shall be worn when changing tong dies; The rotary table shall not be used for final making up or initial breaking out of a pipe connection. Two tongs shal! be used during make up and break out: When excessive pull is needed to break a tight joint, all floor crew shall move away from the rotary and out of the path of the tongs before torque is applied; === dc2021-06-0016-pages-59.ppm === 10. 11. 42. Personne! shall not stand between the two pipe tongs while the driller is making up or breaking out pipe or collars; All pins used to secure chain and jaws shail be kept in place with cotter pins or safety pins; and Power tongs are to be operated by the driller, toolpusher or assistant driller only. E. Rotary Table Area 1. 2. Personnel shail not step on the rotary table while it is rotating; The rotary table area shail be constructed or covered with a non-skid material: Hoses, ropes and lines shall be clear of the rotary table and adjoining rotating equipment while it is in motion; When not in use, the pipe tongs shall be secured away from the rotary table; lf the rotary table is removed, the pick-up lines shall be of sufficient strength and condition and placed evenly around the block hood to ensure a level lift; Rotary hoses shall be equipped with safety lines at both ends; Safety chains shall be used to secure vibrator hoses to an immovable object such as pump base or sub base; and Hoses shail be inspected periodically and maintained to manufacturer's specifications. F. Floors and Walkways 1. All moving machinery that presents a hazard to personnel working in its proximity shall be adequately guarded; Any removed guard shall be replaced before ihe machinery is returned ic operation; Appropriate jock-out and tag-out procedures shall be used before removing guards; All discharge piping openings in floors shall be protected or provided with covers and closed when not in used; === dc2021-06-0016-pages-60.ppm === Guardrails shall be provided for all raised floors and walkways over 1.2meter (3.9 ft) in height or when adjacent to tanks or other hazardous equipment; Non-skid material shall be positioned around the rotary table; and Rig personne! shall not run on stairs, floors and walkways except in cases of extreme emergencies. G. Equipment Guarding 1. Alt moving machinery that presents a hazard to personnel working in its proximity shall be adequately guarded; Any removed guard shall be replaced before the machinery is returned to operation; Appropriate LOTOTO procedures shall be used before removing guards; All discharge piping that could cause burns shall be insulated or guarded; and All portable tools with exposed moving parts shall be equipsed with proper guards. H. Derricks and Masts inspections of the derrick or masts shall be performed periodically, The pipe racking fingers shall be kept straight and secured with a safety device; Drill collar tie back ropes shall be secured to a derrick or mast member; Personnel shail not be in the mast or derrick when pulling or jarring on stuck pipe; The derrickman prior to each trip shall be visually check the derrick or mast escape device; Safety lines shali be cnecked regularly to make sure they are properly secured; The derrickman shall inspect the monkey board prior to its use; and Workers shall not ride ihe mast as it is being raised or lowered. Ali other personnel shall stand clear of the A-frame until the mast radius is in position. 60 === dc2021-06-0016-pages-61.ppm === }. Pipe Racks and Bins 1. 3. 4. When pipe, casing or any other piece of equipment is being moved on pipe racks and in bins, personnel shall not be allowed under, inside or on top of pipe racks and bins; Each layer of tubulars shall be secured to avoid movement; Pipe racks shall be inspected periodically; and Pipe racks and bins shall be level. J. Derrick Escape Line 1. 7. The escape line shall only be attached to non-moveable objects and the landing area kept clear of obstructions at all times. For exampie, do not use a power tong skid as an anchor if it will be moved during casing jobs; The escape line and safety buggy shall be installed before drilling out of the surface casing; The escape line shall be with a minimum of 42mm (1/2") diameter wire rope and shall be tightened to a tension that allows a person descending on the line to touch clear ground between 6-8 meters (20 - 26 feet) from the ground anchor; The safety buggy shall be equipped with a suitable hand brake and shall be kept at the principal working platform (monkey board) and be readily accessible at all times; A visual inspection of the safety buggy shall be performed prior to each trip by the derrickman; The safety buggy shal! be installed according to manufacturer's specifications; and The safety buggy shail only be used in the event of any emergency. K. Drilling Line, Crown Block and Traveling Block 1. Drilling tines shall be visually inspected daily for wear and breaks, in addition to the regular ton-miles inspection; Workers shall use caution when handling the drilling line; The deadline anchor bolts shall be checked periodically; All excess drilling line shall be spooled up off the ground; 61 === dc2021-06-0016-pages-62.ppm === 5. 6. The crown block and traveling block sheaves shall be checked periodically for proper gauge; and A slip and cut program shall be followed. Calculate the ton-miles and follow the slip and cut program as recommended by the drilling line manufacturer. L. Draw Works, Brakes, Clutches, Sand Reel and Wire Line unit 1. 10. 11. 12. All guards shall be in good condition and shall remain in place when the draw works is being operated: Only the driller or assistant driller shall operate the draw works; The tool pusher and maintenance supervisor shall supervise all draw works repairs: Personnel shall not stand on top of the draw works or inside the sand line or main drum guard when the drums are moving; The driller shall chain the brake down anytime he leaves the driller's console; The sand reel shall be equipped with a proper line-spooling device; Defective zerk and alemite fittings shall be replaced; The brake blocks, linkage. pins, cotter keys, etc. shal! be checked periodically; Water shall not be directed at the brake band when washing the draw works: While operating the draw works the operator shail not be distracted: Clutches shall be Kept in good condition and properly adjusted; and All personnel shail be instructed on how to shut down the draw works and rotary table in case of an emergency. M. Mud Pumps and Equipment 4. 2. Caution shall be used when working around high-pressure components; Appropriate LOTOTO procedures shail be used when repairing or maintaining equipment: Mechanically driven pumps shal! have air isolation valve closed and a sign put on driller's console, advising pump is under repair/maintenance; === dc2021-06-0016-pages-63.ppm === Mud pumps shall have pressure relieving safety devices. The devices shall be installed and maintained according to manufacturer's recommendations: Measures to eliminate the use or restrict the operation of the safety device shail not be taken; Liners shall not be removed or seated in a pump by running the pump, applying hydraulic, pneumatic or gas pressure; Guards shail be in place and secured while a pump is in operation; Studs and nuts on the fluid end shall be maintained in good condition and kept tight; and Good housekeeping and lighting shall be maintained around the mud pumps. Mud Tanks and Equipment 1. 40. High-pressure fittings and lines shall be properly rated for the expected working pressures; Mud gun unions and connections shall be kept tight; Only competent personnel shall be allowed to operate mud guns; Flow lines shall be placed to prevent tripping hazards; Electrical mud equipment shall be located to prevent contact with fluids; Caustic soda shal! be added to water slowly to avoid splashing and shall be mixed using a mixture tank or barrel; Approved explosion proof lighting fixtures shall be installed on mud tanks; Handling of chemicals must be in accordance with Section Ill.H hereof, Agitator power source shall be isolated and locked out before entering the mud tanks. Confined space entry permits shail be issued and only suitably qualified personnel may enter; and At the mud mixing hopper or mixing location, the following items shall be available for the workers use. Eye protection; Face protection; Proper respiratory protection; Rubber gloves and apron; A978 === dc2021-06-0016-pages-64.ppm === e. Eyewash station; and f. Appropriate warning/danger signs. O. Pressure Lines and Fittings 1. ~l 10. All pressure lines and "flexible" joints shall have safety devices attached to prevent line whip in case of rupture; Personnel shall familiarize themselves with the location of high-pressure lines and fittings; Personnel shall not attempt to tighten or loosen hammer unions or other connections under pressure; Pressure release valves shall be checked periodically according to manufacturer's recommendations; Lines or piping systems that may kick under pressure shall be secured: Lines under pressure shall not be hammered on; High-pressure lines shall be fitted with high-pressure fittings. Caution shail be exercised curing repairs to ensure that only high-pressure fittings are being installed; Ail personnel shall stand clear of high pressure lines being used during cementing, acidizing, well testing procedures, etc. ; Chicsan sections in high-pressure lines shall be watched for leaks. Caution shall be used during movement; and A pre-operation safety meeting shall be held before unusual or non- routine jobs begin. Pressure Vessel Work Permits shall be issued. P. Engines 1. Positive lock-out measures shall be provided to ensure that the source of power is not activated during engine repair, inspection or adjustments; All exposed revolving parts such as radiator or cooling fans, belts, flexible drives, generators, water pump pulleys, shafts, couplings and other moving parts shall be provided with adequate guarding to prevent contact; Guarding removed for maintenance purposes shail be replaced as soon as possible and prior to operation of the equipment: Spark arresting devices shall be installed in engine exhaust systems where required: 64 === dc2021-06-0016-pages-65.ppm === ™“ Engines shall be equipped with safety alarms and/or automatic shutdown controls to be activated during emergencies or operational difficulties such as over-heating, low oil pressure and over-speeding; Exhaust manifolds and piping shall be constructed, installed and maintained to prevent exhaust gases from leaking between the engine and discharge line. The discharge line shail be directed away from the engine and the work area: Areas below engine skids and beneath the sub base shail be kept clear of drained motor oil and filters; The air box drains, on engines so equipped, shall be drained according to the manufacturer's recommendations; and Personnel working in high noise areas shall wear hearing protection. Q. Air Operated Equipment 1. nN All air receivers (tanks) shall have their maximum pressure limitations stenciled on them; Air tanks shall be drained frequently to prevent accumulation of moisiure in the absence of auto drain; A valve shail not be installed between the tank/compressor and the relief valve if it cannot be isolated: Compressed air used for cleaning purposes shall be regulated to limit pressure; Each airline shall be secured to prevent it from coming loose at the connections; and Compressed air or pressurized air shall not be used as a means of dusting off personnel. R. Blowout Prevention (BOP) Equipment 1. All repairs to BOP equipment shall be documented and supervised by the toolpusher/driller; Pick-up lines, eyes and shackles shail be inspected for strength and condition prior to lifting a blowout preventer; Blow out preventer and components shall not be climbed on until they are set on the wellhead flange, the test stump, the flange of another blow out preventer unit, in the stowed position or on the spider beam and at least 4 securing bolts installed and tightened; 65 === dc2021-06-0016-pages-66.ppm === While guiding or aligning the blow out preventer into place, personnel shall ensure their hands and feet are kept from between the flanges and not placed on top of the boits that have already passed through the holes of an upper flange, while the unit is being lowered; While hammering up the blow out preventer bolts and nuts, only closed socket hammer wrenches of correct size, as well as sledge hammers of suitable weight and size shall be used. Hydraulic torque equipment shai be used according to manufacturer's recommendations; A rope shall be used on the hammer wrench to hoid it in place when hammering; Personnel shall not be permitted on the blowout preventer when the stack is being pressure tested; and When nippling down the blowout preventer, all hydraulic lines shail be held to zero pressure before hammering is permitted on the hydraulic unions. S. Bop Accumulators, Pulsation and Suction Dampeners 1. 5. All repairs to the accumulator shall be documented and supervised by the tool pusher, Accumulators and high-pressure pulsation dampeners shall be charged with nitrogen by qualified personnel as per the manufacturer's recommendations. The pre charge shall be checked frequently; Each contro! on the BOP accumulator and each BOP remote unit shall be clearly marked as to its exact function: Suction dampeners shall be charged by qualified personnel as per manufacturer's recommendations; and Oxygen SHALL NEVER be used to pre charge any pressure vessel. T. Stabbing Board Stabbing board locking devices shail be provided so that: a. One locking device operates when the lifting handle is in neutral: b. One locking device operates if the hoisting mechanism fails; and c. Both devices are independent of each other. U. Tubular 1. Joints of drill pipe that are to be added to or removed from the drill string shall not be allowed to remain in the mouse hole while hoisting or lowering pipe during a trip; 66 > === dc2021-06-0016-pages-67.ppm === 11. 12. 13. 14. 15. 16. Each stand of drill pipe being hoisted from the rig floor shall be held back as the pipe is lifted, so it shall not swing freely across the rig floor; The driller's view of the drill pipe in the rotary table shall not be obstructed at any time; Pipe shall be racked by pushing against the outer face of the pipe when setting it back. Feet shall be kept away from beneath the pipe as it is set down, Feet shall not be placed beneath the pipe when rabbiting drill pipe; When rabbiting tubulars, crewmembers shall face away from the pipe rack to avoid possible eye injury, even though eye protection is being worn during this operation; Each drill collar lift sub shail be tightened into the collar before the coilar is hoisted into the derrick to prevent the sub backing out while working with the drill collar; Feet, knees and hands shall not be placed on the underside of a drill collar clamp while the clamp is being tightened onto a drill collar in the rotary table; A drill collar clamp secured to a drill! collar shall not be hoisted overhead height into a derrick: 10. When setting the drill bit into a bit breaker, hands and feet shal! not be placed on the bit breaker while the bit and collar are guided into the bit breaker opening. Feet shall not be used to adjust the lock on the bit breaker at any time; Stabilizers, wall scrapers or pup joints, being added into the drill string, shal} not be set on the rotary table unsupported by a hoist line; Tools removed from the drill string shall not be thrown or allowed to fall onto the rig floor. They shall be handied with a hoist line; Formation mud shali be scraped off the drill collards and stabilizer blades before the collars are hoisted and stood in the derrick; Personnel shail not peer through the rotary table when the BOP is operated: Casing equipment shall not be rigged up until the drill pipe has been hoisted out of the well bore; The casing tong safety door shall not be removed; this will ensure hands and limbs are protected from the tong's drive works; 67 === dc2021-06-0016-pages-68.ppm === 17. 18. 19. 20. A work scaffolding or platform shall be constructed around the casing spider to prevent having to over stretch or become off balance to reach the equipment; Hands shall not be placed on top of the casing collar (joint) protruding from the casing slips when another joint of casing is being stabbed. Hands shail be kept on the outside of the casing at all times; Single joint elevator shall be used to pick up casing and shall have a safety pin attached by chain; and When removing cores from a core barre! on the drill floor, personnel shall not place their hands or feet under the core barrel opening. V. CHEATER BARS 4. Consideration shall be given to the strength of the component (boit, spindle, etc.) to be turned and a cheater bar shall not be used ifa “TWIST OFF" is considered possible; Cheater bars on handles, wrenches, etc. constructed of cast or welded materia! should not be used: The operating length of any tool/handie should not be extended by more than 100%; The option of changing the tool for a larger one is to be considered prior to use of a cheater bar; Cheaters are to be used in a pushing motion with body stability maintained by adopting a wide legged stance; The position should not be between the tool/nandle and any obstruction, trip hazard or drop: and Care shall be exercised that the bar does not slide on the tool/handle being operated. Section Vill — Separability Clause In the event that any provision of this Department Circular or the application of such a provision to any person or circumstance is declared invalid, the remainder of the Department Circular and the application of such a provision to other persons or circumstances shall not be affected by such declaration. Section IX = Resolution of Conflicts and Overiapping Jurisdictions In case any provision of this Department Circular conflicts, duplicates or overlaps with rules and regulations being implemented by other government agencies, 68 === dc2021-06-0016-pages-69.ppm === such conflict, duplication or overlapping shall be resolved by coordination or any other means of cooperation among such agencies. Section X — Repealing Clause All geothermal safety and health rules and regulations, orders or parts thereof which are inconsistent with or contrary to this Department Circular are hereby repealed, amended, or modified accordingly. Section X! - Effectivity This Department Circular shall take effect fifteen (15) days following its publication in the Official Gazette or in two (2) newspapers of general circulation.
